Qualified/ Senior Social Worker for the Adult Generic Team
Job Description
As a Social Worker in the Adult Generic Team, you will play a pivotal role in assessing the needs of adults with varying levels of vulnerability and supporting them to maintain independence and well-being. This role involves conducting person-cantered assessments, planning care, and coordinating services to address a broad range of needs, from physical health and mental well-being to social support. You will work closely with adults, their families, and other professionals to ensure comprehensive care and support within the community.
Responsibilities include:
Conduct holistic assessments of adults under the Care Act 2014, covering physical, mental, and social needs to develop personalised care plans.
Develop and implement care plans that promote independence, enhance well-being, and address specific needs in areas such as housing, health, and social care.
Provide early intervention and prevention services to reduce risks of deterioration or crisis, supporting adults to remain in their homes and communities.
Collaborate with multidisciplinary teams including health professionals, housing services, and community organisation’s ensure integrated and coordinated support.
Identify safeguarding concerns and implement appropriate actions to protect adults from abuse, neglect, or harm.
Review care plans regularly to adapt to changing needs and circumstances, ensuring ongoing relevance and effectiveness.
Advocate for adults’ rights, ensuring they access the necessary resources, benefits, and services to lead fulfilling lives.
Ensure compliance with statutory duties, including safeguarding procedures, while adhering to best practice standards.
Requirements:
2 years experience post qualification
